All Intermediate School Sports have been postponed until at least September.

Sports Athlete Physicals and Participation Forms:

**PARENTS OF 2020-2021 CCISD ATHLETES** CCISD is moving to online forms and information for the 2020-2021 school year. Please create your account: Rank One Sports Parent Access 

This must be completed prior to first practice or first tryout day.

Your child will not be allowed to practice  or tryout until all the district forms, including a 2020-2021 physical form, are completed through Rank One. The last of the 4 page physical form must be filled out by a physician and scanned to the coach.

Remember, if your child has asthma, the asthma form is also required.


Eagle Families, You are receiving this email because our records show that the back-to-school forms for your student have not been completed.  An important deadline is quickly approaching.  After 3pm on Monday, August 10, you will no longer have the choice to select the Clear Connections (online school) option for your student.  Please complete your Back-to-School forms no later than 3pm,  August 10, 2020, to verify school attendance for class lists and scheduling purposes. Anyone who does not complete the forms will automatically be enrolled in the Brick and Mortar option for the first nine weeks of school. Go to CCISD.net to get the latest updates to the Safely Reopen Plan and Details about the Clear Connections (online) program and the Brick-and-Mortar (in-person) choice that parents and guardians will indicate through the Back-to-School forms.  Link for instructions - To Fill Out/Submit Your Back-To-School Forms: Log in to your Skyward Family Access account and find the forms on the left. Parents who select Clear Connections will receive an e-mail regarding course selection. Parents of both new and returning students will not be required to bring their registration documents to the campus this year. The required documents can be uploaded into Skyward Family Access. Campuses will reach out if there is an issue with the uploaded documents and further verification is needed. Laptop distribution for all Clear Lake Intermediate incoming 6th graders and those 7th and 8th grade students new to CCISD ONLY will be Thursday, August 13, 2020 from 4pm – 7pm in the Clear Lake Intermediate Commons. Everyone entering the building must wear a mask.  Only one adult per student will be allowed into the building.  Please park in the back parking lot accessible from El Dorado Blvd.  Enter through the gate and the door under the breezeway and proceed down the hall to the commons.  The computer must be logged in to with the student ID and password while still on campus the first time.  Please allow some time in your schedule to do that.  After you have a successful log in at CLIS, the student can log in from home.     Please access the sign up by going to the sight below to schedule a time that will work with your schedule. https://www.signupgenius.com/go/60B0944A8AA2EA2FE3-clis You will only be coming into the commons and exiting the building.  The rest of the building will not be open to the public.  If you are not available on the evening of August 13th, you will need to call to schedule an appointment to have a laptop issued to your student.  We will begin to accept appointments on August 14th for the week of August 17th. Insurance for your student's laptop can be paid for in Skyward Family Access.  This insurance covers many of the repairs but does not cover the loss of a charger.  Laptops are traceable to your student but chargers are not.  You have permission to write your student's name on his/her charger with a silver Sharpie or marking pen.  No markings may be made on the laptop but students often slide a piece of paper with artwork or their name under the clear plastic cover.  This can be done without taking the cover off.  The cover should always remain on the student laptop.   If you have questions or concerns, please call the school office at 281-284-3200 Monday through Friday from 8:30am - 4pm.
